When Family H wanted to bring their three year old daughter Rachel home from hospital following her in patient stay since birth, there was obviously a lot of preparation required to make her transition from hospital to the community. The biggest issue was housing, as the family required a disabled access ground floor accommodation with sufficient rooms to accommodate Rachel’s overnight nurse and equipment. Our support worker referred the family to Hackney’s specialist housing department and supported them through the assessment process and assisted with the selection of the property. Rachel is now home, and the support worker has continued to support the family by helping Mum get back to work and Rachel to access a placement at Side by Side, a nursery for Children with Special Needs. The family is now managing independently and have been discharged from our SP service achieving fantastic outcomes in all key areas.
